PARENTAL CONSENT AND RELEASE OF PHOTOGRAPHIC IMAGES

Throughout the school year, New Horizons Homeschool Group will often have candid and posed pictures taken of our adult and student members. These pictures are intended to be used for our yearbook, our website, and/or printed communication such as brochures. Names of adults and students are never used on the website or on printed communications.

In the appropriate area below, please list the names of every adult and child from your family who attends NHHG. Each person should be listed in one area only, based on the photographic usage you consent to.

FULL RELEASE

Permission is granted to use pictures of the following family members for all purposes stated above.

PARTIAL RELEASE

Permission is granted to use pictures of the following family members for general co-op purposes such as the yearbook, but NOT for the website of NHHG or printed communications.

RELEASE DENIED

Permission is denied for the use of any pictures of the following family members for any purpose.
